consent and release form for minor child

CONSENT is hereby given to SchoolPower, Laguna Beach Education Foundation to use photos, video and audio of my minor child, singly and/or in a group and for press, television and other news media information relating to school, athletics and school-related extracurricular activities for the following purpose(s):

SchoolPower or school publicity including but not limited to, SchoolPower Student Directory, SchoolPower website, social media or eblasts, SchoolPower or school event presentations, SchoolPower newsletters, annual reports, brochures, posters or advertising, school publications, and/or press releases.

It is agreed that I shall not have any right, title or interest in any such photos nor any cause of action for damages or injuries relating to or arising out of their use for the purpose(s) set forth herein. I understand that these photos may be used beyond the current school year.
